- removed always empty variable $pCreate_mailbox_username_text
- replaced "static" variables $pCreate_mailbox_password_text and
$pCreate_mailbox_quota_text with their PALANG content in the template

- replaced $tMessage with flash_error() / flash_info()
templates/create-mailbox.tpl:
- added 4th column for error messaegs
- replaced <h3> with <th>
- marked lables with <label>
- aligned button with input fields
- removed tMessage
